چکیده

Dear Editor, We thank the authors for a comprehensive review1 on the role of interventional pain therapies used in cancer pain as well as a informative local audit using the intrathecal catheter (PORT-A-CATH® IT) implantable system (Deltec Inc, USA).2 Our comments are directed at the neuraxial delivery of drugs in both articles. We agree that the most widely accepted indications for intrathecal analgesia in cancer pain are uncontrollable side effects of the systemic pain therapy and inadequate pain relief with parenteral opioids despite dose escalation.3 The authors in the review suggest that epidural analgesia can be used as a trial to assess the effectiveness of pain relief before placement of a permanent implanted intrathecal drug delivery (ITDD) system. On the contrary, we advocate initiating intrathecal analgesia straightaway as intrathecal drug delivery has been shown to produce higher rates of satisfactory pain relief.4 From the practical point of view, this avoids the additional titration step, the more frequent changes of drug reservoirs with epidural infusion and the concomitant risk of epidural fi brosis in the long term. Epidural analgesia infusion is only indicated in

Epidural Infusion of Morphine and Levobupivacaine through a Subcutaneous Port for Cancer Pain Management

BACKGROUND To manage intractable cancer pain, an alternative to systemic analgesics is neuraxial analgesia. In long-term treatment, intrathecal administration could provide a more satisfactory pain relief with lower doses of analgesics and fewer side-effects than that of epidural administration. However, implantable drug delivery systems using intrathecal pumps in Korea are very expensive. Cons...

Intrathecal analgesia in patients with cancer pain--an audit in a tertiary institution.

INTRODUCTION Cancer pain is one of the most frequently encountered pain syndromes. With the application of the World Health Organization analgesic ladder, adequate analgesia is achieved in 75% to 90% of patients. The remaining patients suffer from intractable pain requiring intrathecal analgesia. The aim of this study was to retrospectively analyse the pain intensity before and after intratheca...

Intrathecal betamethasone for cancer pain in the lower half of the body: a study of its analgesic efficacy and safety.

BACKGROUND Sufficient analgesia for cancer pain is sometimes difficult to achieve with conventional treatments. We aimed at investigating the analgesic efficacy and safety of intrathecal betamethasone in patients with uncontrollable cancer pain. METHODS Betamethasone 1 mg mixed with saline was injected into the lumbar intrathecal space once a week in 10 patients with persistent cancer pain in...

Continuous Intrathecal Morphine Administration for Cancer Pain Management Using an Intrathecal Catheter Connected to a Subcutaneous Injection Port: A Retrospective Analysis of 22 Terminal Cancer Patients in Korean Population

BACKGROUND Intrathecal opioid administration has been used widely in patients suffering from severe cancer pain that is not managed with conventional modalities. However, the potential serious neurological complications from the procedure and the side effects of intrathecal opioids have made many clinicians reluctant to employ continuous intrathecal analgesia as a first-line therapeutic option ...

Intrathecal Therapy for Cancer-Related Pain

OBJECTIVE The increasing incidence of cancer survivorship has shifted treatment of cancer-related pain from short-term analgesia to long-term chronic pain management. As a result, alternatives to oral analgesics, such as intrathecal therapy, may be beneficial for patients with cancer-related pain. The authors review the use of intrathecal therapy in the management of cancer-related pain. METH...
